Routes in `pageList` mocks now reflect the way they really look, using a leading slash. Also cleans up FormPage/ReviewPage unit tests which had remnants of vets.gov user login. Fixes #177

Looks like 5459373 also wiped the formConfig.consoleSubmit option as well. Since I had just added that mainly for the hackathon I am fine with leaving it out, you can just take it off the list of changes for the release. Since we now know that the pre-push hook wasn't doing what we want it's not a problem that it's missing in the release. I'll make a ticket for discussion on how to fix that separately.
