Roadmap STU3 to R4

Roadmap

We are in the process of transitioning to R4 as our primary version of FHIR:

  • Baseline and freeze ci-fhir-stu3
  • Transition to R4
    • Establishment of HL7 AU Base Implementation Guide materials incl profiles, extensions - national level localisation and agreement of the representation of health information
    • Transition to R4 of Agency clinical informatics core resusable models that tailor the national level structures to support multiple implementation usage scenarios
    • Development and publication of draft R4 Agency implementation guides for one or more implementation usage scenarios
  • BAU in ci-fhir-r4

The above stages are not sequential.

Baseline and freeze ci-fhir-stu3 - intended to be completed by January 2020

Primarily the remaining work in ci-fhir-stu3 will wrap-up current profiles and implementation guides into a cleaner STU3 baseline, addressing known defects that can be practicably actioned in the near term, and then freeze.

Planned scope:

  • Alignment of standard draft informative material (Guidance, Conformance, Downloads, etc.) in the following FHIR implementation guides - Shared Health Summary, Event Summary, Personal Health Records, Advance Care Records, and Prescription and Dispense Record Lists.
  • Correction of known defects in STU3 profiles:
    • Correct invariants with absolute paths to be relative
    • Add missing invariants on the Reference type, e.g. invariant on Condition to constrain the references of Condition.recorder & Condition.subject
    • Correct fixedCodeableConcept instances to be fixedCode and fixedSystem on code.coding
    • Ensure profiles are FORGE compliant
  • Addition of known issues to the following FHIR implementation guides - Shared Health Summary, Event Summary, Personal Health Records, Advance Care Records, and Prescription and Dispense Record Lists.
  • Clean-up of draft usage scenario and implementation guidance in the following FHIR implementation guides - Shared Health Summary, Event Summary, Personal Health Records, Advance Care Records, and Prescription and Dispense Record Lists.
  • Correction of defects in examples used in the following FHIR implementation guides - Shared Health Summary, Event Summary, Personal Health Records, Advance Care Records, and Prescription and Dispense Record Lists.

Transition to R4 - intended to be completed by Dec 2020

  • Investigate and establish new template-based publishing for FHIR-IGs
  • Collaboratively develop a set of R4 HL7 AU structures that are complete, correct, consistent, and sufficiently support known localisation requirements - this is not a 'port' it will re-examine the context, requirements, and design opportunities. The rough order of development is expected to be:
    • Patient, Practitioner, PractitionerRole, Device, Organisation, RelatedPerson, AU Address
    • Composition (this may not be required), Medication, MedicationStatement, Dosage
    • Encounter, AllergyIntolerance, Condition, Procedure, Immunization, Location, HealthcareService
    • Specimen, Diagnostic Observation, ImagingStudy, DiagnosticReport
    • Medicines Lists, Allergy Lists, Problem Lists, Medical History Lists
  • Transition to R4 Agency clinical informatics reusable models that tailor the national level structures to support multiple implementation usage scenarios - this is not a 'port' it will re-examine the context, requirements, and design opportunities. The rough order of development is expected to be:
    • 'Base' of Patient, Practitioner, PractitionerRole, Device, Organisation, RelatedPerson, Medication, MedicationStatement, Specimen, Location, HealthcareService
    • 'Summary' of AllergyInterolance, Procedure, Condition,
    • 'Event' for encounter, medicines list
  • Development and publication of working draft R4 Agency implementation guides for one or more implementation usage scenarios. The scope of STU3 FHIR-IGs to be transitioned are: Shared Health Summary, Event Summary, Shared Medicines List, and Personal Health Records.
    • Compositions of Event Summary, Shared Health Summary, Personal Health Summary, Personal Health Notes, Practitioner Authored Shared Medicines List

BAU in ci-fhir-r4 - intended to be largely in this stage in August 2020

  • Active membership in HL7 AU working groups continuing to evolve and mature the set of HL7 AU base structures on an ongoing basis via direct contribution, QA and testing of peer materials, design discussions, etc.
  • Continuous development of Agency clinical informatics resusable models is occurring in support of Agency work plans
  • Reusable models are assembled using context grouping structures like List / Composition / Encounter profiles and published in wording draft implementation guides
  • Work is progressing on priority usage scenarios:
    • Broaden the SML FHIR Implementation Guide to support additional usage scenarios
    • Support exchange of diagnostic imaging reports
    • Support exchange of pathology reports
    • Support exchange of discharge summaries
    • Supporting a co-authored, shared, living medicines list
    • Supporting a co-authored, shared, living allergies lists
