Question About Missing Observations in Custom Module

First of all, thank you for such a useful tool.
I have a question regarding custom modules. I created a module where I want to get a list of resources at the end. However, I found that observations are not created every time.

For example, for 100 patients, only 8 observations are generated. Why is that? Other resources, such as conditions and encounters, are created every time. I can't find any information about this.
