[chore] header filter improvements

Description

Adds contextual error messages to log entries where requests have been blocked due to header filtering. This makes it much easier seeing what exactly is getting blocked specifically by the header filtering system. Also removes the currently unused stats being gathered on header filters, but leaves all the TODO messages / logic in place for integrating it with OTEL in the future.
