02/04/15 - Fixed lzham_decompress_reinit() - it was not resetting the arithmetic table statistics, this was causing the 7z-LZHAM decompressor codec to fail on solid block 1

Also reduced the time taken by lzham_decompress_init() a small amount.

02/03/15 - Speeded up decompression by up to 1.5-2% by adding a Huff decode accel table for the distance LSB symbols
