dwc_otg: make channel halts with unknown state less damaging #340

If the IRQ received a channel halt interrupt through the FIQ with no other bits set, the IRQ would not release the host channel and never complete the URB. Add catchall handling to treat as a transaction error and retry.

In section_deactivate(), pfn_to_page() doesn't work any more after ms->section_mem_map is resetting to NULL in SPARSEMEM|!VMEMMAP case. I'd be interested to see if this squashes the bug you have with your XBMC/NFS timing out. The error message will still be printed but should no longer break ethernet/etc.
I forced lots of spurious channel halts during testing and the USB system seemed to keep working.
