Refactored internals and api to reach stable state

I introduce some breaking changes to push this project finally into a stable state. There are breaking changes applied to the internal code to achieve:

• a better code structure
• simplify the code base (remove FunctionApply and ApiCompilerOps with shapeless.ops.function)
• renamings to increase readability

And a small change in the api:

• for the client:

// single api
compile(Api) --> derive(Api)

// list of apis
compile(Api) --> deriveAll(Api)

// now you have to pass the ClientManager directly
val foo = derive(Api)

foo(...).run[IO](cm)

• for the server:

// single api
link(Api).to[IO] --> derive[IO](Api).from

// list of apis
link(Api).to[IO] --> deriveAll[IO](Api).from
