COVID-19 Assessment form

• A built-in form to capture COVID-19 vaccination and infection data
• Its accessible through the Triage or Clinician App on homepage
• Alerts available to guide clinical decisions
• The form is listed under available forms during an active visit

Subsequent visit

• Vaccination status for fully vaccinated client
• Vaccination status for a partially vaccinated client

Defaulter Tracing Form

• Inclusion of COVID-19 reasons for missing appointment

Discontinuation Form

• COVID-19 Complications added under specific cause of death

COVID-19 Report

• COVID-19 Report has been added under the Reports - HIV section
• Missed Appointment report due to COVID-19 – under common Reports

Facility Dashboard

• Additional section on COVID-19 available
