Disable Cancel appointment button after cancellation | Facility switcher hovering and some minor fixes

Task 1 : Cancel appointment button is active even after cancellation in appointment details dialogue and use detructive variant for cancelled badge

Task 2 : Facility and Organization switcher on hovering appearing gray it should be green

Task 3 : Incorrect icon for Nearby facilities. It should be d-hospital icon

Task 4 : User profile pic is visible but Username is not visible for symptoms and diagnosis table in encounter page

(3) Alright make the change, but in general avoid suggesting any changes for icons for now because the sidebar items are all in flux.
(4) is already being handled in #10017.
