Towards an EBNF grammar

Many people would like to see an EBNF grammar created for AsciiDoc. In order to get there, some of the ambiguities need to be cut from the syntax. These cuts will be good anyway since it will eliminate some of bad practices used by writers and generally make AsciiDoc cleaner.
