EHR - On patient page, last visit is N/A even if the patient has had a visit

Acceptance criteria

• Last visit' and 'Last office' columns are populated with date and Office name when appointment is in 'checked-out' status
• Last visit' and 'Last office' columns have '-' values when appointment isn't in 'checked-out' status, e.g. 'ready for provider'/'ready for discharge' status, etc.

@GiladSchneider Note: For PM QRS 'Last visit' and 'Last office' columns should be populated with date and Office name only if visit is in 'checked-out' status. Is this requirement applied for Ottehr as well?
