Migrates to version 2.0 of the ClinicalTrials API

Adds functional support to v2.0 of the API, still has a lot of features missing.

The study_count endpoint is no longer available, so the corresponding function has been removed.

The names of the fields for the CSV format has changed so now we validate CSV and JSON study_fields differently.
