0.1.4 - Form fixes and better new user experience

This release updates the form fields to allow users to edit phone and email fields immediately. Previously, they had to exist in the pod already to edit, and could not be created.

Some minor code refactoring, including additional comments around email/phone to explain what's going on.

Also fixed a bug where inrupt provider link was pointing to the wrong place.
