Standardize UI on Bootstrap 3

Mainly anything not covered in berkeleybop/bbop-js#26.
This is actually a reasonably small thing now that the accordion has been switched--there are maybe two more smaller widgets used in the non-labs stuff that would need to be changed, and a few output pages. But mostly, we're just talking about updating tables and a few pop-up widgets.

This item technically means the rewrite of the LiveSearchGOlr.js app, but is likely not as bad as it sounds since most of the logic code can be saved, although the buttons and pager will have to be differently weird than they are.

I think I'm down to just pop-ups and cleaning the spinner interactions (may need to port over the pre/post hooks from the better kltm/go-mme managers).

Made improvements, but completion will have to wait until 2.3.

Please revert the fix for #187 and make it a more generally palatable solution.

Time is a little tight on the 2.4 release, so even though a bunch of progress was made, I don't think this is going to happen here. Kicking to 2.5.
