[telemetry] Add TelemetryPrometheusExporter livebook example

Add a Livebook example that shows how export all metrics that :grpc_prometheus currently exports.
This is so we can avoid maintaining a separate repository as well as forcing an optional dependency version, which can also bring some maintenance overhead.
