Support querying patient resources by managingOrganization and optionally partOf

modify repository subject provider to allow for Patients to queried by related managingOrganization reference, in addition to optional partOf related Organizations. partOf logic should only be triggered if optional configuration (which defaults to false) is set to true.

this should allow Care-gaps, evaluate-measure, and evaluate-measures to execute subject=Organization/{id} and optionally also pull in related org linked patients

DSTU3 is out of scope
