Removes bootstrap manifests from the installer and uses the
kube-core-renderer image to render them instead.

Bonuses:

  • Bumps hyperkube to 1.9.1
  • Cleans up a bunch of unused variables

AWS, Azure and Metal all tests passed. Running only on Govcloud now.
