By default, the first issuer that returns a certificate will be used. (This is unchanged.)
If UseFirstRandomIssuer is configured, then the list of issuers will be shuffled each time before using the first one that returns a certificate.
This implementation is experimental and may change later. It's for partial completion of #214.
