VISIT_DETAIL postgresql DDL OMOP 5.3

Hello

I need the new postgresql schema with visit_detail new tables and indirect modification. I am going to make the modification myself (if this does not allready exist for sure).

Moreover, I may add the field & table comments in order to help users. (I guess I will just create a program that take them from the markdown tables wiki pages)

Will you be interested in a pull request or something then ?

I think your contribution would be welcome. I updated the documentation in this PR that was merged today. #102
Note- the modification applies to many tables including the clinical event tables like condition_occurrence, procedure_occurrence etc.

Yes, all the details were exposed in #70
Then I have it working (only ddl & comments)
I also made many minor patch to documentation where I found typos
