Internal links and TOC links not working when the document URL has %-encoded characters

文書のURLに％エンコーディングされた文字があるとき、内部リンクやTOCパネルからのリンクが機能しない不具合がある。

This bug did not exist in v2.1.1. The following change in v2.1.2 has problem and need fix again:

• Internal links not working when the URL fragment has %-encoded characters (f12e9c5), closes Internal links not working when the URL fragment has %-encoded characters #649
