TOC is not enabled when TOC exists in HTML but is not specified in the manifest

Vivliostyleで本を作ろう Vol.4 のように、複数のHTMLファイルからなる本の場合、

のように、目次要素（<nav role="doc-toc">）が存在する最初のHTMLファイルを指定した場合には目次パネルが有効になる（CLIの場合は、「しおり」付きのPDFが生成される）が、次のように manifest ファイルを使用した場合には有効にならない:

https://vivliostyle.org/viewer/#src=https://vivliostyle.github.io/vivliostyle_doc/ja/vivliostyle-user-group-vol4/manifest.json&bookMode=true

この manifest.json 内に 目次が存在するファイルを "rel": "contents" で指定すれば目次を有効にすることはできる。

しかし、その指定が省略されていても、最初のHTMLファイル内に目次がある場合にはそれを有効にするべきである。
