Publish statistics to AMQP exchange

The SSE Hub could publish the number of connected clients on each of its channels every X seconds (configurable).

If we publish to the same exchange/queue that the hub is consuming, we get the benefit of automatically broadcasting it on all SSE Hubs consuming the channel. For this reason, we probably should include the hostname+port or similar.
