Unable to pair Xbox with iPhone

The logs are telling me to connect manually:
[1/12/2022, 10:31:58 PM] Please add [Xbox One S 7E1E] manually in Home app. Setup Code: 000-00-000
Setup code was redacted btw,

When I do that, it sits at "Connecting to Set Top Box..." for like 2 minutes then throws an error saying it couldn't connect:
Unable to add Accessory, Home couldn't connect to this accessory

Please help lmao
