Modal formatting is slightly broken with silverstripe/admin 4.12 #480

Some of the visual formatting has broken with silverstripe/admin 4.12 - I presume this is a result of updating jquery-ui.

Steps to reproduce:

Go to /admin/workflows/
Click through to edit a workflow
Select a new action in the dropdown and click "Create"

You can substitute step 3 for editing an action, or adding/editing a transition - basically anything that opens the modal.

Difference between 4.12 and 4.11

Note the x icon in the top right corner, and the resize icon in the bottom right corner.
