50c-FAT not loading

What is your media type (HDD, USB Flash, SD/SDHC)?

How is your media formatted (wbfs, fat32, ntfs)?

Which cios are you using, which version, which base (222,249,...)?
(Please try both 249 and 222 with cfg-222.dol)

all i am truly aware of is 
~it used to work just fine
 ~it wont load past the cfg title screen
  ~this used to happen when it was in the incorrect usb port
  ~no matter what usb port i put it in this happens
