Minor UI Fix in the add patient Pop-up at patient tab

Describe the bug

the pop-up coming after we click the add patient button in the patient page are following issue :

• the box , head and buttons are not properly aligned
• the pop-up seem really big, in normal view
• the droplist is overflowing the data outside the page size in normal view

Expected behavior

improve the droplist along with size of the box and alignment
