A way to approve or reject the reply without having to click on all three yes/no buttons.

I think that's an excellent idea that all moderators/reviewers & admins would appreciate. Sometimes the reply can be immediately identified as very good or unacceptable. Let's make it easier for us to approve & reject those types of replies. If we're concerned about accidental approvals/rejections, we can create a page that lists the approved/rejected reviews for manual moderation with a "Undo Approval/Rejection" option.
I was going to suggest adding a additional "Are you sure you want to approve/reject this?" prompt but that still adds to the "too many mouse-clicks to do one task" problem we have. So I don't think we should add that but instead periodically review the future "Approved/Rejected Responses" page for accidental approvals/rejects.
Extra Brainstorming/Bonus Points:
If we're worried about lower quality approvals/rejections, we could limit these "power" buttons to specific trusted users for each locale. A admin could enable it for them from a Admin Panel or Settings area.
