Issue Type: Feature Request

I'd like to use the breadcrumb as a replacement for the Outline and the Open Workspace Symbols. Therefore I'd like to rebind Cmd+Shift+O. This should focus the breadcrumb and open/expand it at the same time.

VS Code version: Code - Insiders 1.26.0-insider (5fae91d, 2018-07-25T01:56:56.548Z)
OS version: Darwin x64 16.7.0
