Multi selection does not work

toUpperCase and all those functions are usefull for more than markdown files ...

I think you should enable this plugin for all files ...

In fact I'm not sure but it seems that it's already enable for all files, my bad ! It's weird, 4 hours ago I couldn't use the uppercase feature within my javascript file but now I can ... :/

But another issue is that it seems to not works with multi selectors. If I select a word and use ctrl + d to select all occurrence, only the first one is changed when I choose toUpper.

All fixed up sorry about that.

I also cleaned up several other little bugs :)

@dranc have a try now and see if v1.0.0 is better for you and thanks for reporting the issue. cLosing for now :)
