Create and link pdfs for consent forms

HIPAA Acknowledgement and Consent to Treat and Guarantee of Payment links should open the corresponding placeholder pdfs

Currently empty pages are open when user clicks links on Complete consent forms from Intake flow

Description:
Given that a user has started the intake flow and has reached the Complete Consent Forms screen, when the user clicks on the HIPAA Acknowledgement link or Consent to Treat and Guarantee of Payment link, empty pages are opened instead of the proper links or PDF files.

Steps To Reproduce:

1. Start the intake flow as a patient.
2. Navigate to the Complete Consent Forms screen.
3. Click on the HIPAA Acknowledgement link or Consent to Treat and Guarantee of Payment link.

Actual result:
Empty pages are opened when the user clicks on the links.
