When the contents of an illustration holder paragraph contains square brackets [], the resulting epub cannot be opened and the resulting pdf has 'missing.jpg' scattered throughout.. as if the brackets are being interpreted as part of a regex.

Noting in issue 154 that this was encountered again, closing this duplicate issue
