[8] Footnote in chapter title errors

Non-urgent (this project won't work in Bookmaker for other reasons), but at least we're tracking it. This MS has a footnote at the end of a chapter title, which causes an epubcheck error. Likely because in the epub, the chapter title is a link to the TOC, but the footnote number is also a link.
