Caitlin Buckley was trying to make an ebook excerpt of a single chapter, and the heading is suppressed. She said she can fix this one manually on the ebook, but as we get more requests for teasers/excerpts we should make this optional. Manuscript and resultant epub attached.

This will be fixed as part of macmillanpublishers/bookmaker_addons#121 . The requirement will be that users will need to manually add the "Begin Reading" chapter title in a Nonprinting title element.
