Routing problem: Controller is mapped to handle requests to /activityProviders, which is never made since index.html links to /providers. Reopening issue.

Suggestions:

1. Rename view from activityProviders.html to providers.html and update accordingly in the controller (return providers and redirect to /providers);
2. Fix url in index.html to the providers view, changing the href from /providers to /activityProviders.
