Implement an “Other” access policy

NADA currently has the following option for data access policy (applies to microdata):

• Open
• Direct
• Public
• Licensed
• External
• Enclave
• No access

Add an option “Other” which will allow catalog administrators to enter their own text (terms of use) and provide links/forms.
