Ex.1 Output has no grey dot in 3x3 black in the middle
Ex.4 Output has one

Is it possible to solve this one 100% of the time without knowing how to place this dot?

I too think this is is an error. The solution should show the Grey, Red, Grey, Black, Grey .... pattern diagonally into the center with the consecutive color due in the center node (when it there is one).
