MV/Index fill that tolerates leadership changes

Currently we cancel an MV or index fill if a leadership election occurs concurrently. This is because fill must occur on the processor and leadership election causes processor to migrate and we can't currently pause and resume an MV fill.

We should come up with another way of filling which allows processor migration to occur.
