Improve performance of CDK toolkit #241

We should profile and identify performance improvement opportunities in the toolkit. Things that come to mind:

Storing default account ID on disk (Store default account in disk cache #220)
Avoid multiple invocations of the CDK app (cxapi 1.2 #160)
