"Shipping Information Form" - some drop downs and toggle checkbox not working

I'm new to Semantic UI. I followed the set-up instructions for Semantic then created a index.html page containing the code for "Shipping Information Form" from https://semantic-ui.com/collections/form.html.

Issue#1 - When I click on the down arrow for the following drop downs, the list doesn't appear:
-ui fluid search selection dropdown
-ui selection dropdown
-ui fluid multiple search selection dropdown

Incidentally, other drop downs in the form work.

Issue #2 - When click on the "ui toggle checkbox", the control doesn't slide out or change colour.

I really like the interface and would like to solve these issues so I can use it.

Any advice would be appreciated?
