Featured image doesn't get updated if changed

When a revision to a post comes along, sometimes that includes a replacement featured image -- usually for rights issues. The plugin logic currently won't update the '_post_thumbnail' post meta key in that case, and besides that ignores the more modern 'set_post_thumbnail()' approach.

A specific example that made me notice this is this story https://www.npr.org/2022/06/23/1106944327/deb-haaland-indian-boarding-schools -- the original post_thumbnail didn't get updated though the new featured image did get sideloaded and added to the media library.
