Expose a location for extensions to add to the main menu bar

Sometimes, some extensions would has a lot of commands. It is not convenient for users to use key bindings or Command Palette. Therefore, it would be great that vscode could expose context menu api in header, so extension could put dozens of command in context menu of toolbar like PlatformIO for Atom
