Smart step should be dynamically toggleable - when I debug with it enabled, I have to stop to disable it too often.

@roblourens Is there an indicator in the UI as to whether or not smartStep is on? I verified that the toggle command shows up in the context menu, but it would be nice to know what the current state is before pressing it

If there is a file in the callstack that is skipped by smartStep, then it's grayed out, and has a tooltip "Skipped because of smartStep". That's it unfortunately
