Create a CSV version of CmdExtractor

This sounds nice but isn't very practical as it would require a separate output CSV file for every different command... @jasonatball @donaldatball thoughts?

I would like a CSV output like the following:

Time, Target, Command, Parameters
2018/07/12 16:45:17.413, INST, COLLECT, 0, 1, 0, ...
2018/07/12 16:46:18.629, INST, ABORT, 0, 1, 0, ...

Obviously you'd have to do some additional parsing or lookup if you want to know what all the parameters are but many times you just want a command timeline and I think this goes a long way to providing that.
